GERD management is far more simple than before. 

Just upper GIT endoscopy done by Dr. 

Badawi in addition to manometry if needed can early assess the medical treatment needed or if this is associated by hiatal hernia, surgical treatment is an early option that can prevent further complications. 

Hiatal hernia procedures are widely practiced by Dr. Badawi with promising results. This can be done by complete or incomplete wrap of the stomach after crural repair as is pre-operatively assessed.

Moreover, Post operative follow up is crucial for theses patients as follow up endoscopy is usually needed. 

On the other hand, Achalasia of the cardia can be present this can be further evaluated by radiology which demonstrates difficulty of passage of food or fluids to the stomach because of extreme fibrosis at it’s inlet. 

Achalasia of the cardia is not uncommon condition. This condition has wide variety of treatment options, what is commonly practiced by Dr. Badawi is the laparoscopic Heller’s-Dor technique as this removes associated fibrosis within the stomach (not done by endoscopic technique as POEM). Meanwhile this way partial anterior fundoplication can prevent GERD from happening as we cut through OGJ, this is also not done by endoscopic techniques.
